eXtend Program PATIENT ACCESS PROGRAM

The Pfizer eXtend Program patient access program aims to support rheumatoid arthritis patients for enhanced compliance to their treatment through the following program features:

PROGRAM MECHANICS
  1. Patient to complete the patient enrollment form (manual or link/QR Code) and email to the eXtend Program Hotline. Patient may request for the unique eXtend code from his/her doctor.

  2. The eXtend Counsellor will contact the patient within 24 hours upon receipt of enrollment (Mon-Thurs) and on the first business day if submitted on a weekend . Patient shall receive a call from the eXtend Counsellor to discuss the program details and inform of the next steps. Next steps include submission of additional requirements.

  3. Patient to submit requirements to the eXtend Program email address:
    • A valid prescription, which is one whose prescribed number of units is more than or equal to the lapsed number of days from prescription date. (Example A - Valid Prescription: Prescription Date = Jan 1, 2022; Request date= April 1, 2022. Equivalent # of days 90. Units in Rx=90. Example B - Invalid Prescription: same dates as above but unit in Rx= 60. 60 is less than 90 days, thus invalid.)
    • Valid government ID (ex. driver’s license, SSS, voters ID, passport, Pag-Ibig, GSIS, PHILSYS, senior citizen)
    • Financial supporting documents:
      1. 1)  Latest ITR or latest payslip and
      2. 2)  latest electric bill or other similar documents. If unemployed, Barangay Certificate of No Income/Indigency. Barangay Certificate should include the patient’s address.
    • Travel documents as needed for more than 2 months request.
  4. eXtend Counsellor will review the requirements and assess eligibility. Once deemed eligible, the eXtend Program Hotline will send the patient a unique Patient ID and eXtend code to avail from the participating pharmacy which will be communicated by the eXtend Counsellor.

  5. Patient proceeds to the participating pharmacy to present the unique Patient ID, eXtend code, prescription and government-issued ID and purchase the medicine.

  6. For medicine refill, patient will contact the eXtend Program Hotline to request for a new eXtend code. Please ensure that a valid prescription is available. Once a new eXtend code is provided by the eXtend Program Hotline, proceed to step 5.
